»	Online submission is just for pre-registration. To get complete membership please follow the procedure given on membership page.
»	This form needs to be accompanied by a scans of your NIC and students ID card (if applicable) and other documents required to verify education and experience.
»	Payment to be made by Pay order / bank draft in favor of "Pakistan Nutrition & Dietetic Society" or cash on receipt.
	Annual Membership covers a period of 12 months from 1^st July of current year to 30^th June of next year. The subscription rates may be revised by the PNDS as or/and when the General Body deems fit.
